<div><em><strong>Waleed Tawfiq</strong> asked: </em><br/><br/><br/><strong>Evidence based case study in management of acute </strong><strong>Exacerbations of COPD.</strong><br/><br/><strong>Introduction:</strong><br/><br/>Chronic pulmonary diseases have become increasingly one of the most common chronic lung diseases and a major cause of morbidity and mortality in modern world. It is characterized by airflow limitation that is not fully reversible.<br/><br/> Chronic Obstructive Disease is a leading cause of the death in the worldwide (Calverley et al, 2003). The condition can result in loss of work quality and quality of the life can be significantly effected (Barnes, 1999). In UK 27,478 men and woman die because of the chronic obstructive lung diseases and most of the death ( more than 90%) was in  the age of above 60 years old(British Thoracic Society. 2006).<br/><br/>Rehabilitation for patients with chronic lung diseases is well established and widely accepted as means of enhancing standard therapy in order to improve symptoms and maximise the patients function (Siafakas et al, 1995; Ries, 1990; Casaburi, 1993; Fishman, 1996). In 1974, the American College of Chest Physicians (ACCP) focused in there definition of Pulmonary rehabilitation on three important features and they suggested that Successful pulmonary rehabilitation depends on three importance features, Individuality of each case, Multidisciplinary team approach and attention to physiopathology and psychopathology of each case.<br/><br/>One of the main problems with COPD patient is the increase in the pulmonary secretions leading to increase in shortness of breath. These two factors affect the patient’s function and quality of life.  For exacerbation, Physiotherapy is often required to help clear secretions and reduce WOB, including non-invasive ventilation to prevent intubation (Alexandera, 2001).<br/><br/>There are various techniques, which can be used in physiotherapy to improve patient’s condition. The research suggests that the postural drainage is beneficial in clearing  the chest from secretions (Clarke,1989;Faling,1986), respiratory muscle relaxation manoeuvre is effective for improving the pulmonary function of pulmonary emphysema patients (Fujimoto et al, 1996), relaxation can help reduce dyspnoea and anxiety in chronic obstructive pulmonary disease (COPD) patients (Louie, 2004).<br/><br/><strong>Case description <img src='http://respiratorytherapyschoolsonline.com/wp-includes/images/smilies/icon_sad.gif' alt=':(' class='wp-smiley' />  case history, physical examination, and intervention)</strong><br/><br/>Patient is a 67-years-old woman with acute exacerbation in Chronic Obstructive Pulmonary Disease (COPD). She complained of increased shortness of breath with loose, non-productive cough. A febrile on auscultation, bilateral rales, rhonchus, and expiratory wheezing. Patient said she is on bronchodilators and low-dose steroid. Patient said she has been suffering from this problem since 10yrs and has been on medication since. She does not do any exercises and her general practitioner who she usually sees has never mentioned about seeing any physiotherapist. Recently during this episode of acute exacerbation, she was advised by the hospital doctor to see a physiotherapist.<br/><br/>The strategy in this case study used was the problem-solving model, which included following six steps;<br/><br/>Step 1: Patient assessment,<br/><br/>Step 2: defining the problem,<br/><br/>step3: determining the goals,<br/><br/>step4: identifying appropriate techniques,<br/><br/>Step 5: applying the techniques,<br/><br/>step6: re-evaluation of the patients situation(Donna,1987).     <br/><br/><strong>Evaluation and assessment:</strong><br/><br/>Accurate assessment is the key player of physiotherapy and forms the bases of rational practice. A Problem based assessment leads to reasoning in the pulmonary rehabilitation. As result, a thoughtful evaluation will guide to both effectiveness and efficiency  because time will be saved by avoiding unnecessary treatment (Physiotherapy in Respiratory Care An evidence-based approach to respiratory and cardiac management).<br/><br/>Ward reports and medical notes of the patient were evaluated to know about;<br/><br/>·         The past and present relevant history.<br/><br/>·          social history , accommodation<br/><br/>·         Conditions required precautions in relation to certain treatments e.g. light-headedness ,bleeding disorders or swallowing disorders<br/><br/>·         Recent cardiopulmonary  resuscitation to examine the X-ray in case of gastric aspiration or fracture<br/><br/>·         Checking for possibly of bony metastases, long-standing steroid therapy that this leads to risk of osteoporosis and checking for the history of radiotherapy over the chest. These all findings contraindicate percussion or vibration over the ribs.  <br/><br/>·         The patient’s experience increased shortness of breath and the assessment indicate airway secretion.<br/><br/>A part of the patient evaluation was subjective assessment and that was by listening to patient’s problem in her own words. Following symptoms were checked:<br/><br/>Respiratory symptoms by looking for the how long the symptoms been troublesome.<br/><br/>·         Frequency, duration, and the severity.<br/><br/>·         Any pain, chest pain, musculoskeletal pain or cardiac pain.<br/><br/>·         Checking functional limitations including the daily living.<br/><br/>·         Observation to check the breathing rate and pattern before the patient a ware of the physiotherapist’s presence to avoid any role-play.<br/><br/>·         General appearance , colour, hand checked which is a good and rich source of information like cold hand indicate a poor cardiac output, oedema, jugular venous pressure, chest shape.<br/><br/><strong>Objective measurement: </strong><br/><br/>Exercise testing was used to monitor the progress of the patient due following few reasons:<br/><br/>·         Lung function tests are not a good predictor of exercise capacity (Bradley et al, 1999).<br/><br/>·         The laboratory tests are for physiological measurement rather than monitoring of patients progress.<br/><br/>·         The patients own estimate of exercise tolerance is not objective (Hough, 2001).<br/><br/>Exercise testing:<br/><br/>As long as the patient was not suffering from acute breath illness, exercise testing was used as an objective measure to monitor the progress. Oximetry on exercise testing was used which is advisable to measure the level of oxygen during the exercise (Martine et al, 1992). Because the patient was in acute exacerbation condition, only simple stair climbing testing was used and count the number of steps can be climbed up and down in 2 minute and rest allowed but included within the 2 minutes.  Each minute was passed the patient was informed about the time. The result of the test was only 10 steps per 2 minutes. Exercise testing revealed increased shortness of breath and from assessment of patient, it was clear that she had airway secretions.<br/><br/><strong>Defining the problem:</strong><br/><br/>Shortness of breath was probably due to increased secretions with the patient and so physiotherapy was planned after the use of bronchodilators. Percussion can trigger bronchospasm in patients with asthma and in this case would benefit to have maximum bronchodilator prior to treatment. (Donna 1987)<strong></strong><br/><br/><strong>Determining the Goals:</strong><br/><br/>Promote airway clearance; encourage relaxation and breathing exercise; encourage exercise to promote airway clearance.<br/><br/><strong>Identifying Appropriate Techniques:</strong><br/><br/>   Due to short shortness of breath, modified positioning was used for postural drainage as per the patients comfort, turning the patient side to side to prevent any shortness of breathiness (Hough 1991). Trendelenberg position was also used with percussion and vibration was gently applied due to consideration that the patient was on long-term steroid therapy. Emphasis was placed on both lower lobes as no specific area of pathology was described. Relaxation exercises were done for upper chest and neck to increase the ventilation, abdominal areas. Patient was also taught home postural drainage to help in early recovery.<br/><br/>Walking and cycling was encouraged, as it is most widely used modalities of exercise training in chronic obstructive pulmonary disease rehabilitation (Vallet et al, 1997). Patient was given endurance (aerobic) training program for 4-12 weeks (Casaburi et al, 1997; Wijkstra et al, 1996), and she attended supervised training sessions 2-5 times a week. Each session duration was 20-30 minute.   <br/><br/><strong>Applying techniques:</strong><br/><br/> Techniques for vibration and postural drainage (Gumery et al 2001) were applied with consideration to the contraindications and patients condition and motivation. As the patient was on long term of cortico- steroid treatment, possibility of osteoporosis was considered which may led to fracture while doing tapping in postural drainage.<br/><br/><strong>Re-evaluation:</strong><br/><br/>The patient was re-evaluated after the secretions were mobilised and on observation, patients breathing was found to be more effective. The progress was slow as the patient was reconditioned. Patient was encouraged to remain active to help in early recovery. Patient and family was given education about restoration and maintenance of exercise tolerance and basic self-management. Home visits were made to check for adequate heating, and health or safety hazards. In addition, this visit was also supportive for the family.<br/><br/>The exercise was prescript for the patient to keep the patient fit and increase the vital capacity. The mode of the exercise was related to the patient’s life style and the patient was encouraged to use stationary bike. The bike was suggested as it supports 85% of the body weight, and large muscle groups can be exercised with less strain than walking (Bach and Haas, 1996, p.309). Furthermore, exercise programs for the muscles of ambulation were prescribed as they are a part of virtually every program of pulmonary rehabilitation (Ries, 1990; Casaburi, 1993; Carter et al, 1992; Olopado et al, 1992). Over the period of rehabilitation, the patient also said that her functional capabilities improved and this helped her to great extent in her ambulation. Exercises were also given for muscles of the shoulder girdle as these muscles can help provide support to pull on the ribcage (Criner et al, 1988). Patient was encouraged to resume her sports hobbies – bowling to combine exercise and recreation.<br/><br/> Patient was scheduled for a follow up appointment after 6 weeks of rehab and treatment to monitor the patient’s progress. (Broussard 1979; Fujimoto et al. 1996; Gift, Moore, and Soeken, 1992; Louie, 2004). Patient was also provided with breathlessness rating scale to check her breathlessness after each session of exercise.<br/><br/>On the follow up appointment subjective and objective re assessment was done. Patient as observed to check the breathing pattern and frequencies, auscultation was done to check the chest for any signs of secretion and obstructions. The patient was sent to take x-ray to check the clarity of the chest. Stair climb test was done and there was a good progress in the patient’s condition as the result was increased significantly from 10 steps in 2 minute before 6 week to 25 step.<br/><br/>Improvement was also seen on the self reported and measured breathlessness rating scale where the patient scored 2 whereas she scored 4 during initial assessment and also the recovery rate post exercises reduced from 5-10mins to 2-5mins and the patient also reported that she was doing fine the day and was comfortable.<br/><br/><strong>Summary:</strong><br/><br/>From the above case study, it can be derived that patients suffering with similar conditions can benefit from appropriate exercise and active lifestyle. 			<content:encoded><![CDATA[<p>Respiratory Therapy as known today did not exist 50 years ago. The field has grown  <img title="respiratory therapy schools 1" src="http://tbn0.google.com/images?q=tbn:gUJwE5FmHEJ9dM:http://iwcc.cc.ia.us/programs/departments/images/clip_image002_014.jpg" alt="" width="109" height="111" align="right" />greatly since a few oxygen tank technicians began meeting with doctors concerned with lung disease in Chicago, Illinois in the 1940s. This group named itself the Inhalational Therapy Association (&#8220;ITA&#8221;) in 1946. They gradually put together a series of classes for people administering medical gases to patients. In December, 1950, 31 members of the Association were issued certificates for attending 16 lectures, the first certification of &#8220;inhalation therapists.&#8221; (Burton, pp. 5-6) This was an on-the-job training system for so-called &#8220;oxygen jockies&#8221; with little formal education, but a desire to do their jobs better and help patients in the process.</p>
<p>In 1953, the American College of Chest Physicians agreed to sponsor the ITA. By this year, the ITA was holding an annual convention and showed attendees from 14 states and Canada. In 1954 they changed the named to American Association of Inhalation Therapists, elected a board of directors. They elected Sister Borromea as president and appointed Jimmy Young as executive director. By this time the AAIT was putting out a monthly Bulletin, and state chapters began to form. (Burton p. 7)</p>
<p>&#8220;During the next few years better equipment and more advanced therapy techniques were introduced. Those practicing in the profession began to adopt the title &#8220;inhalation therapist.&#8221; Their main function was to ensure safe oxygen use, to administer intermittent positive pressure breathing (IPPB) treatments, to perform cardiopulmonary resuscitation (CPR), and to operate negative pressure (iron lung) ventilators.</p>
<p>Alvin Barach, a leader in discovering the role oxygen therapy played in breathing and health, led a group in publishing in 1950 &#8220;Minimum Standards for Inhalation Therapy.&#8221; In 1957 the New York delegation to the AMA&#8217;s House of Delegates introduced a resolution to develop schools of inhalation therapy, and soon after some schools were set up. In 1960 Articles of Incorporation were filed with the State of Illinois for the American Registry of Inhalation Therapists. Twelve examinees were registered in 1961. In 1972 the Association changed its name to the American Association of Respiratory Therapy, and this was later changed to the American Association of Respiratory Care. By 1983 about 16,000 respiratory therapists had been registered. (Burton, pp. 9-10). About 1969 the organization devised a system for credentialing entry-level practitioners, which became the National Board of Respiratory Care&#8217;s system of certifying technicians.</p>
<p>With the advent of positive pressure mechanical ventilators, the more widespread hospital provision of neonatal and pediatric care, more sophistocated pulmonary function testing (PFT), a need for thoroughly trained clinical practitioners presented itself.</p>
<p>Over the years &#8220;respiratory technicians&#8221; have evolved to being college and University trained personnel who mainly administer inhaled medications. Skills that Respiratory therapists must master include, but are not limited to; intubation, other advanced airway placement, arterial-line insertion, Cardiac Catheter advancement, tracheostomy re-cannulation, naso-tracheal suction, Blood Gas (drawing and interpretation), Waveform analysis, Inhaled medication delivery, other aerosol therapy devices, mechanical ventilation (invasive and non-invasive) among others.</p>

About 79 percent of jobs were in hospitals, mainly in departments of respiratory care, anesthesiology, or pulmonary medicine. Most of the remaining jobs were in offices of physicians or other health practitioners, consumer-goods rental firms that supply respiratory equipment for home use, nursing care facilities, and home health care services. Holding a second job is relatively common for respiratory therapists. About 12 percent held another job, compared with 5 percent of workers in all occupations.</p>
<p>&nbsp;Faster-than-average employment growth is projected for respiratory therapists. Job opportunities should be very good, especially for respiratory therapists with cardiopulmonary care skills or experience working with infants.</p>
<p>Employment of respiratory therapists is expected to grow <span>19</span> percent from 2006 to 2016, faster than the average for all occupations. The increasing demand will come from substantial growth in the middle-aged and elderly population&mdash;a development that will heighten the incidence of cardiopulmonary disease. Growth in demand also will result from the expanding role of respiratory therapists in case management, disease prevention, emergency care, and the early detection of pulmonary disorders.</p>
<p>Older Americans suffer most from respiratory ailments and cardiopulmonary diseases such as pneumonia, chronic bronchitis, emphysema, and heart disease. As their numbers increase, the need for respiratory therapists is expected to increase as well. In addition, advances in inhalable medications and in the treatment of lung transplant patients, heart attack and accident victims, and premature infants (many of whom are dependent on a ventilator during part of their treatment) will increase the demand for the services of respiratory care practitioners.</p>
<p><strong><em></em></strong>Job opportunities are expected to be very good. The vast majority of job openings will continue to be in hospitals. However, a growing number of openings are expected to be outside of hospitals, especially in home health care services, offices of physicians or other health practitioners, consumer-goods rental firms, or in the employment services industry as a temporary worker in various settings.</p>

			<content:encoded><![CDATA[<p>An associate degree is required to become a respiratory therapist. Training is offered at the postsecondary level by colleges and universities, medical schools, vocational-technical institutes, and the Armed Forces. Most programs award associate or bachelor’s degree and prepare graduates for jobs as advanced respiratory therapists. A limited number of associate degree programs lead to jobs as entry-level respiratory therapists. According to the Commission on Accreditation of Allied Health Education Programs (CAAHEP), 45 entry-level and 334 advanced respiratory therapy programs were accredited in the United States in 2006.</p>
<p>Among the areas of study in respiratory therapy programs are human anatomy and physiology, pathophysiology, chemistry, physics, microbiology, pharmacology, and mathematics. Other courses deal with therapeutic and diagnostic procedures and tests, equipment, patient assessment, cardiopulmonary resuscitation, the application of clinical practice guidelines, patient care outside of hospitals, cardiac and pulmonary rehabilitation, respiratory health promotion and disease prevention, and medical recordkeeping and reimbursement.</p>
<p>High school students interested in applying to respiratory therapy programs should take courses in health, biology, mathematics, chemistry, and physics. Respiratory care involves basic mathematical problem solving and an understanding of chemical and physical principles. For example, respiratory care workers must be able to compute dosages of medication and calculate gas concentrations.</p>
<p>A license is required to practice as a respiratory therapist, except in Alaska and Hawaii. Also, most employers require respiratory therapists to maintain a cardiopulmonary resuscitation (CPR) certification.</p>
<p>Licensure is usually based, in large part, on meeting the requirements for certification from the National Board for Respiratory Care (NBRC). The board offers the Certified Respiratory Therapist (CRT) credential to those who graduate from entry-level or advanced programs accredited by CAAHEP or the Committee on Accreditation for Respiratory Care (CoARC) and who also pass an exam.</p>
<p>The board also awards the Registered Respiratory Therapist (RRT) to CRTs who have graduated from advanced programs and pass two separate examinations. Supervisory positions and intensive-care specialties usually require the RRT.</p>
<p>Therapists should be sensitive to a patient’s physical and psychological needs. Respiratory care practitioners must pay attention to detail, follow instructions, and work as part of a team. In addition, operating advanced equipment requires proficiency with computers.</p>
<p>Respiratory therapists advance in clinical practice by moving from general care to the care of critically ill patients who have significant problems in other organ systems, such as the heart or kidneys. Respiratory therapists, especially those with a bachelor’s or master’s degree, also may advance to supervisory or managerial positions in a respiratory therapy department. Respiratory therapists in home health care and equipment rental firms may become branch managers. Some respiratory therapists advance by moving into teaching positions. Some others use the knowledge gained as a respiratory therapist to work in another industry, such as developing, marketing, or selling pharmaceuticals and medical devices.</p>

			<content:encoded><![CDATA[<p>Respiratory therapists are responsible for evaluating and treating patients with breathing or lung disorders. Working together with, and often under the supervision of, physicians or medical professionals, respiratory therapists treat disorders such as asthma, emphysema and cystic fibrosis using a variety of methods and highly-specialized equipment to assist patients with their breathing. To evaluate patients, respiratory therapists interview them and conduct diagnostic tests such as measuring their breathing capacity and determining the concentration of oxygen or acidity in their blood. To treat patients, respiratory therapists set up and monitor ventilator equipment, perform chest therapy to remove mucus from their lungs, and assist them with rehabilitative exercises. Although most respiratory therapists work in hospitals, an increasingly large number of them are employed by home care agencies, nursing homes, and respiratory therapy clinics.</p>
<p>The minimum educational requirement to become a respiratory therapist is an associate degree, although a bachelor&#8217;s or master&#8217;s degree is highly preferable. There are literally hundreds of accredited respiratory therapy programs available today. They are offered at colleges and universities, medical schools, vocational/technical institutes, and the Armed Forces.</p>
<p>In all states except Alaska and Hawaii, respiratory therapists must be licensed in order to practice. Licensure is usually based on obtaining the Certified Respiratory Therapist (CRT) credential awarded by the National Board for Respiratory Care (NBRC). In order to earn CRT certification, a therapist must have graduated from an accredited educational program and must also pass the licensing exam offered by the NBRC. An advanced credential, the Registered Respiratory Therapist (RRT) certification, is available to CRTs who have graduated from advanced programs and are able to pass two separate examinations.</p>

<p>The medical field has since broadened. There are now fields of specialization so if you haven&#8217;t yet chosen what you want to pursue, perhaps you should browse through the medical a list of medical careers. To give you some idea, here are a few of them :</p>
<p>1.Dentists</p>
<p>Brushing and flossing your teeth isn&#8217;t enough to keep them clean. This is why people are told to visit the dentist once a year or every six months. Dentistry is usually completed after 8 years of education. It takes just as long as a medical doctor and analysts believe this demand will grow as the large number of are expected to retire soon.</p>
<p>The job of the dentist is to diagnose, prevent and treat problems in the teeth and also the mouth tissue. They may perform x-rays, put in protective plastic sealants, extract or repair fractured teeth. If patients need to straighten their teeth, then they have to visit an orthodontist.</p>
<p>Dentists use a variety of equipment to get the job done. These include mouth mirrors, probes, forceps, brushes, scalpels, drills and x-ray machines. To protect themselves and their patients, they must always sterilize their equipment; wear masks, gloves that must be disposed afterwards and protective glasses.</p>
<p>2.Emergency Medical Technicians</p>
<p>When someone is injured, we are taught to dial 911 and wait until help arrives. The one that comes through the door is a EMT or emergency medical technician whose job is to stabilize the patient until they reach the hospital.</p>
<p>The EMT uses special equipment like a backboard to immobilize the patient if they are seriously injured to the regular stretcher. They always work in teams. One EMT drives the ambulance while the other stays in the back to monitor the patient. Some EMT&#8217;s are even part of the flight crew of a medical evacuation team.Once they reach the hospital, they have to inform the doctor the current situation and then let them take it from there.</p>
<p>3.Nurses</p>
<p>If you like to work in a hospital or clinic side by side with a doctor, then perhaps you can choose to become a practical nurse. Their job is to care the convalescent, disabled, injured and sick. This is done by making the rounds and checking on the vital signs of the patient including the blood pressure, pulse and respiration.</p>
<p>They will also administer injection, enemas, monitor catheters, treat bedsores, give massages, apply dressings and a host of other things as instructed by the doctor. If another test is needed, they will collect samples. Depending on what state they are in, they may be allowed to prescribe medicines or start intravenous fluids.</p>
<p>4.Optometrists</p>
<p>If you want to help someone with their eyesight, you can become an optometrist. Their job is to examine the vision of the patient and then prescribe eyeglasses or contact lenses. They may also prescribe medication as part of vision therapy and low vision rehabilitation.</p>
<p>People should not be confused between the optometrist and the ophthalmologist because this person does not more than just hand out corrective lenses. They perform eye surgery so the person may see again like those who are suffering from glaucoma and other eye diseases.</p>
<p>5.Respiratory Therapists</p>
<p>You may also become a respiratory therapist. This person is tasked to evaluate, treat and care for patients who have difficulty breathing or are suffering from other cardiopulmonary disorders.</p>
<p>They usually work under the direct supervision of the physician. To evaluate a patient, an interview is first conducted before a physical examination is done. Tests are then done to analyze the oxygen, carbon dioxide and pH levels and then comparing them with the age, height, sex and height of the patient if they are below or above normal. The results are then used to make the proper treatment decisions.</p>
<p>Respiratory therapists do not only treat old people but also premature infants that have not yet produced fully develop lungs. They may also be called in to help those suffering from asthma, drowning, heart attack, shock and a stroke.</p>
<p>6.Social Workers</p>
<p>One job that is not often see in line with health and medical careers but do play a role is that of a social worker. They help people who have serious illnesses, disabilities and substance abuse. They also assist families who have inadequate housing and those who are in serious domestic conflicts especially those with child or spousal abuse.</p>
<p>7.Speech Therapist</p>
<p>People who have a hard time speaking or pronouncing words will feel better after they are treated by a speech language pathologists better known as a speech therapists. These individuals assess, diagnose and treat those who have speech and communication disorders.</p>
<p>These speech problems may be caused by a brain injury, cerebral palsy, developmental delays, hearing impairment, language difficulties or a stroke. Given that each patient is different, the specialist will have to develop an individualized plan of care in order for it to work. The end result is for the person to be able to make sounds, improve their voice and most importantly increase their language skills so they are able to communicate more effectively.</p>
<p>8.Surgeons</p>
<p>For those that want to get into the heat of the action, they may work as surgeons. This is very challenging as most doctors work more than 60 hours a week inside the operating room. Aside from general surgery, there are many who specialize in a specific area.</p>
<p>Some examples of these include orthodontic surgery that is the treatment of the skeletal system, neurological that is involved with the treatment of the brain, orthopedic that is with the eyes, otolaryngology that is the treatment of the ear, nose and throat as well as plastic or reconstructive surgery for those are recovering from burn injuries or simply want to change their physical appearance.</p>
<p>There are many opportunities in the medical fields. You just have to find something you like and then take the proper courses in order to be certified.</p>
<p>While some of the jobs mentioned you can do for the rest of your life, there are others that you can&#8217;t.But if you still want to help people, go through the above list again and make that career shift.</p>
